New Student Health Insurance Form
RMCAD requires that all students enrolled in 9 or more credit hours carry health insurance coverage.

We offer a comprehensive HMO health insurance plan for those students without their own coverage. The plan is administered through Anthem Blue Cross and Blue Shield of Colorado. Please download and review the health insurance plan description for details. All students without their own coverage are automatically enrolled in our HMO and fees are assessed accordingly.

All new students must complete the New Student Insurance Form before enrolling for their first term.

The New Student Health Insurance Form serves as an acknowledgement of your health insurance plans for your first year at RMCAD. You will either acknowledge that you will be enrolled in the College-sponsored health insurance plan, or indicate that you have your own external health insurance coverage and do not wish to be enrolled on the College's plan. Please note that proof of your own health insurance is required if you choose to opt out of the College's plan.

MMR Immunization Requirement Form
The state of Colorado requires that all students born after 1956 have 2 doses of the Measles, Mumps, and Rubella (MMR) vaccine.

Your immunizations need to be documented on the Certificate of Immunization.

Please complete the top section of the Certificate yourself, then ask your doctor, nurse, or school health authority to complete the rest of the form. The medical professional must mark and attest to the accuracy of the dates of your vaccinations.

Clear and legible official medical records or copies of transcripts which include immunization information may also be accepted to meet the requirement. In this case our Dean of Students, Braelin Pantel, will verify that the requirements have been met.

Students who are immune to Measles, Mumps and Rubella should include the lab test results documenting thier immunity with the Certificate.

Students who do not have immunizations for MMR and choose not to be vaccinated for medical, religous or personal reasons should submit the Certificate with the appropriate exemption signed.

Remember that you must complete the certificate prior to your start of classes at RMCAD.

Meningococcal Information
If you will live in a building with other students we recommend that you receive the Meningococcal vaccine. There is information regarding Meningitis on the reverse side of the Certificate of Immunization.

If you have received the Meningococcal vaccine, please verify that your physician, nurse, or school health authority includes the date of vaccination on the Certificate of Immunization. They can do this while documenting your MMR Requirement.
