Please complete the top section of the Certificate yourself, then ask your doctor, nurse, or school health authority to complete the rest of the form. The medical professional must mark and attest to the accuracy of the dates of your vaccinations.
Clear and legible official medical records or copies of transcripts which include immunization information may also be accepted to meet the requirement. In this case RMCAD will verify that the requirements have been met.
Students who are immune to Measles, Mumps and Rubella should include the lab test results documenting their immunity with the Certificate.
Students who do not have immunizations for MMR and choose not to be vaccinated for medical, religious or personal reasons should submit the Certificate with the appropriate exemption signed.
Remember that you must complete the certificate prior to your start of classes at RMCAD.
Meningococcal Information
If you will live in a building with other students we recommend that you receive the Meningococcal vaccine. There is information regarding Meningitis on the reverse side of the Certificate of Immunization.
If you have received the Meningococcal vaccine, please verify that your physician, nurse, or school health authority includes the date of vaccination on the Certificate of Immunization. They can do this while documenting your MMR Requirement.
